If you would like to instigate a grievance you must follow these procedures.
If you have a complaint regarding any service provider contact ask for their grievance process form.
Inform your case manager regarding your situation. Try to resolve your disatisfaction without going further.
If you do not receive satisfaction to your situation, contact the Ryan White Council for the information
process. State that you have yourself tried to resolve this problem with the Service Organization, and the results of that
attempt. Request a response from the Council stating they have received your complaint letter, and request they investigate
and provide a solution within a reasonable amount of time. If you do not receive satisfaction from these actions, you can
place all that documentation on these pages.
